Schedule: Part-time Saturday 7a-7:30p and Sunday 8a-4:30p; Holiday rotation Location: UChicago Medicine AdventHealth Hinsdale Hospital, 120 N Oak Street, Hinsdale, IL The role you’ll contribute: The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources and qualify for community assistance from a variety of special assistance programs and agencies, and/or require assistance with transitions of care or discharge planning. This role involves crisis intervention, development of discharge plans, and collaboration with interdisciplinary team members to ensure patient-centered care. Responsibilities include patient evaluations, developing and implementing transition of care plans, facilitating communication during multidisciplinary rounds, and ensuring compliance with policies and regulations. The Social Work Care Manager also provides education on resource utilization, manages barriers to care, and advocates for patient and family needs across the continuum of care.
